If you are an employee of Day Kimball Hospital and have Meritain Health, coverage varies depending upon the plan you have subscribed to:

• Network Plan:  This plan has no out-of-network benefits; naturopathic services are not covered.
• Premium Plan:  This plan will cover your visit.  A $15 copayment is required and the claim will be submitted directly to the insurance company.
• Value Plan:  This plan will cover your visit.  A $15 copayment is required and the claim will be submitted directly to the insurance company.

Your insurance card will show the plan you have.  If you can not recall which plan you have subscribed to, contact Human Relations (ext. 2296).
